You're over thinking it.

First, unless it is something very unusual like an obvious stuff gag the chances are the packing will not be in the actresses mouth for 95% of the time that they are shooting. Remember that a few minutes of screen time can take days to film and quite honestly anything that reduces the endurance of your actress is something you would avoid. For much the same reason the actress will be gagged and ungagged off screen multiple times between shots. If the packing doesnt have to be there... then it wont be.

For the scenes that have been mentioned, the "blink and you'll miss it" spitting out of packing during an ungagging. There was probably only one or two takes of that scene and the director will be more concerned with lighting, dialogue and countless other things than how well the gag photographs. The packing is there for realism, because in real life it should be there and if in the take it shows up very obviously then great but if it doesnt they wont refilm the scene to make it look better.

Bollywood Movie
The link is to a review of a Bollywod movie with a line about an actress that says the diretor has her gaged (lierally) almost the whole movie. The picture of the actress that's included shows her to be very good looking.

Karen Duffy has some nice lips
and was gagged in "Memory Run" 1996

blindfolded, cleaved and tied, tied several times, otm gagged..all with the same silk scarf
